Many things affect your health and wellbeing. GPs tell us that a lot of people visit them feeling isolated, lonely or stressed by work, money and housing problems.
Social prescribing is a way of linking patients in primary care with sources of support within the community. It provides GPs with a non-medical referral option that can operate alongside existing treatment to improve health and well-being.
Social Prescribers are available at each General Practice to listen to you, and put you in touch with the people and activities that might help you to feel better.
To request a referral please speak to your GP practice.

Please note that we can only accept referrals from patients from the following Surgeries: Addingham Medical Centre, Dyneley House Surgery, Grange Park Surgery, IG Medical (Ilkley Moor Medical Practice), Ilkley & Wharfedale Medical Practice, Ling House Medical Centre, Townhead Surgery.
